Responsibilities

• Provide school-based case management and care coordination to children, adolescents, and families.
• Build positive, supportive relationships with students and families.
• Assist with assessments and treatment planning with licensed clinicians.
• Identify needs that may affect a student's well-being and connect to services.
• Coordinate care with therapists, school personnel, healthcare providers, families and community agencies.
• Advocate for students and families navigating behavioral health and community resources.
• Complete screenings, intakes, referrals, and documentation; monitor progress.
