The Village for Families &Children in the Greater Hartford region is seeking a Juvenile Justice Case Manager to coordinate care for LYNC youth and families, working with service specialists and community providers. This full-time position emphasizes risk-needs-responsivity, trauma-informed care, and collaboration with schools, courts, and community organizations to connect youth with vital services.

Qualifications

• Education: Bachelor’s in Criminal Justice, Sociology, or Human Services or HS diploma with 3 years in justice/system youth work.
• Experience: minimum 3 years with justice/system/agency involved youth.
• Skills: excellent oral and written communication; strong community connections; organized and independent; culturally competent.

Responsibilities

• Provide direct services to youth and families through care coordination.
• Develop and implement RRSP with youth and family.
• Discharge planning and follow-up with community services.
• Maintain documentation and reporting per policies and funding.
• Collaborate with community organizations to connect youth to services.
• Travel to various sites in greater Hartford and New Britain as needed.
