Job summary

Copa Health in Peoria, AZ is hiring Full-Time Case Managers to develop and implement integrated assessments and ISP plans for members. You will coordinate with medical, behavioral health, and wellness providers, maintain documentation, and support members toward recovery goals.

The role requires a bachelor’s degree in a behavioral health field, or related experience, and bilingual English/Spanish is preferred. Comprehensive benefits, PTO, and retirement plans accompany this position.

Qualifications

  • Bachelor’s degree in behavioral health field and Copa Health's Case Manager Trainee Program; or
  • Bachelor’s degree in unrelated field with at least one year of direct care experience in behavioral health; or
  • Associate degree in behavioral health with two years of direct care experience; or
  • High school diploma with four years of direct care experience in behavioral health.

Responsibilities

  • Engage, educate, communicate, and coordinate care with service participants, family, providers, and resources as per ISP.
  • Develop and maintain the Integrated Service Plan (ISP) within required timelines.
  • Participate in the development of integrated assessments and ISP updates.
  • Implement services per the plan and provide ongoing care and progress reviews.
  • Coordinate with healthcare and behavioral health providers and document progress for recovery goals.

Starting Pay: $22.50-$24.00
Who We're Looking For

We’re seeking Full-Time Case Managers to join our dynamic healthcare team. The Case Manager participates in the development of the integrated comprehensive assessment and Integrated Service Plan (ISP) and implements services to members in accordance with their plan and provides ongoing care and reviews progress. Coordinates with all involved healthcare, behavioral health and wellness providers and maintains current documentation on members’ progress in managing chronic medical and psychiatric conditions and assists the individual in achieving their recovery goals.

Key Responsibilities
  • Works collaboratively with the integrated care team to engage, educate, communicate, and coordinate care with service participants, their family, behavioral health, medical and dental providers, community resources and others in ensuring that all services prescribed in the Integrated Service Plan are implemented.
  • Develops and maintains and manages the Integrated Service Plan (ISP) within required time-frame.
  • Participates in the development of the integrated comprehensive assessment and Integrated Service Plan (ISP).
  • Implements services to members in accordance with their plan and provides ongoing care and reviews progress.
  • Coordinates with all involved healthcare, behavioral health and wellness providers and maintains current documentation on members’ progress in managing chronic medical and psychiatric conditions and assists the individual in achieving their recovery goals.
  • To ensure compliance & adequate services, additional job duties may be required to meet the needs of the program and or department
Qualifications
  • Bachelor’s degree in behavioral health field plus completion of Copa Health's Case Manager Trainee Program; or
  • Bachelor’s degree in unrelated field and at least one (1) year of full-time direct care experience in behavioral health; or
  • Associate degree in behavioral health related field and two (2) years of full-time direct care experience in behavioral health experience; or
  • High school diploma or high school equivalency diploma and four (4) years of full-time direct care experience in behavioral health.
  • AZ valid Driver’s License, vehicle with state minimum insurance coverage and able to meet Copa Health company driving requirements and safety sensitive requirements.
  • Safety sensitive is any job that includes tasks or duties that the employer in good faith believes could affect the safety or health of the employee performing the task or others
  • Bilingual (English/Spanish) preferred.
