BronxWorks seeks a Case Manager to join an established multidisciplinary team in assisting formerly homeless residents. You will provide intensive case management, referrals for medical and psychiatric care, and support services to help clients stabilize and transition to permanent housing.

The role requires a Bachelor’s degree in social work or related field, strong communication and organizational abilities, and flexibility for some evenings or weekends.

Qualificações

• Proficiency in modern business communications including email, voicemail, and office tech.
• Bachelor’s degree in social work or related field.
• One year of case management or related experience preferred.
• Strong written and oral communication, time management, and organizational skills.
• Proficiency in Microsoft Office and standard office technology required.

Responsabilidades

• Maintain a caseload of 14–18 individuals with chronic homelessness history.
• Monitor and document client progress toward treatment plan goals.
• Conduct regular case conferences with clients and service providers.
• Perform monthly home visits for caseload and field work as needed.
• Assist with managing walk-in clients and directing services.
