Job summary

New Destiny Housing Corporation seeks a full-time Case Manager to provide on-site services at The Kira, a 163-unit service-enriched affordable housing development in Jamaica, Queens. You will manage a caseload of 20–25 tenants, coordinate with a team, and connect families to resources to support stability and thriving children.

The role requires one late night per week, strong case management and trauma-informed approaches, bilingual Spanish/English preferred, and a commitment to ending

Qualifications

• Bachelor’s degree required; BSW or BA/S in Psychology, Sociology, or Early Education preferred.
• Knowledge of social services in Queens is preferred.
• 1–3 years direct service experience preferred.

Responsibilities

• Provide case management for 20–25 tenants, including counseling and crisis intervention.
• Develop programming with the Program Director and families; create annual wellness plans.
• Coordinate referrals to housing stability, health, child services and community programs.
• Maintain data in internal databases and attend weekly supervision with the Program Director.
• Engage families to monitor progress and facilitate access to schools, clinics, and providers.
