Job summary

Job Overview – Medical Case Manager (Supportive Housing)
Compensation: $27/hour
Location: Brooklyn, NY
Schedule: Monday to Friday or Tuesday to Saturday, 9:00 AM – 5:00 PM

TAG MedStaffing is hiring a Medical Case Manager (Supportive Housing) in Brooklyn, NY for our client, supporting single adults experiencing homelessness and mental health challenges within a residential housing program. This role provides case management, service planning, wellness education, and housing stabilization support to promote recovery, independence, and successful community living. The position also coordinates access to community resources and monitors residents’ progress toward individualized goals.

Responsibilities
  • Case Management: Provide individualized support to help residents maintain housing, build independent living skills, and access appropriate services.
  • Service Planning: Collaborate with residents to establish rehabilitation and housing goals and maintain current support plans.
  • Resident Engagement: Build supportive relationships with residents and encourage participation in services that promote recovery and self-determination.
  • Wellness Education: Facilitate group meetings focused on wellness management, community living, and personal development.
  • Housing Inspections: Assess residential living spaces, address identified concerns, and report housing conditions according to program requirements.
Qualifications
  • Education: Bachelor’s degree in Human Services, Social Work, Psychology, Sociology, or a related field is required.
  • Experience: 2+ years of experience supporting individuals with mental illness, housing instability, homelessness, or related behavioral health needs is required.
  • Industry Knowledge: Knowledge of supportive housing, community-based services, recovery-oriented care, and housing stabilization practices is required.
  • Technical Skills: Proficiency with Microsoft Office, electronic documentation, and word-processing systems is required.
  • Skills & Attributes: Strong written and verbal communication, case documentation, organization, relationship-building, and problem-solving skills are required, and bilingual English and Spanish proficiency is preferred.
