Job Overview – Mental Health Case Manager

Compensation: $25/hour

Location: Bronx, NY

Schedule: Sunday to Thursday, 9:00 AM to 5:00 PM

TAG MedStaffing is hiring a Mental Health Case Manager in the Bronx, NY for our client, supporting community-based behavioral health services. This field-based role includes conducting psychosocial assessments, developing service plans, completing home visits, providing crisis intervention, and coordinating care with healthcare and housing providers. Ideal candidates have experience in community mental health case management and care coordination within high-need populations.

Responsibilities

• Assessment & Service Planning: Conduct comprehensive assessments and develop individualized service plans with long- and short-term goals.
• Direct Case Management: Maintain weekly client contact (including monthly home visits) and assist with medical care access, medication adherence, daily living skills, and housing retention.
• Crisis Intervention & Support: Provide crisis intervention and ongoing support to promote client safety and stability.
• Coordination & Advocacy: Coordinate referrals to medical, mental health, and social service providers and advocate for client needs, including education on budgeting and tenant responsibilities.
• Monitoring & Documentation: Track client progress, maintain detailed case notes, and complete required documentation, schedules, and supervision.

Qualifications

• Education: Bachelor’s degree in Social Work, Public Administration, Psychology, or a related field required.
• Experience: 2+ year of case management in mental health or community-based settings required, preferably with clients with mental illness or substance use.
• Technical Skills: Basic proficiency in Microsoft Office (Excel, Word) and electronic documentation systems.
• Skills & Attributes: Strong communication and counseling skills, ability to manage multiple priorities, and a compassionate, culturally sensitive approach to field-based work with diverse populations.
• Language Skills: Bilingual in Spanish and English strongly preferred.
