Job summary

Volunteers of America Los Angeles is seeking a Case Manager Specialist I to provide intensive case management for eligible clients and coordinate essential services. You will meet participants weekly and drive to home visits, coordinating housing, health, and social supports.

This on-site role requires strong assessment skills, advocacy, and collaboration with a multi-disciplinary team to help clients achieve goals and access VOALA programs.

Qualifications

• Fingerprint clearance and background check required.
• Valid California Driver’s license and reliable vehicle for home visits.
• Ability to manage stress and work with diverse clients in need.

Responsibilities

• Conduct comprehensive screenings to collect information for case plans.
• Provide service linkages and support systems to meet identified needs.
• Care Coordination and case management of assigned caseload within the team.
• Coordinate planning with clients to meet short- and long-term goals.
