Job summary

The Town of the City of Champaign seeks a dependable Case Manager to coordinate services for shelter guests. You will serve as the first point of contact, manage a caseload, schedule intakes, and ensure safety with the support of staff.

You will document case notes, complete 8-week programming, and participate in community events and external programming, contributing to a compassionate and client-centered program.

Qualifications

• Three (3) or more years of experience in social service entities or a combination of education and experience.
• At least 1 year of social service experience.
• Active certifications or willing to be trained for: CPR & First Aid, Mental Health First Aid, Narcan administration, Trauma Informed Care

Responsibilities

• Manage a caseload between 12-15 guests.
• Meet with guests on caseload weekly or biweekly.
• Approach guests where they are and build goals and programming with them.
• Schedule intakes for guests as requested.
• Ensure client safety and determine next steps if a danger is identified.
• Assist guests with immediate case management needs and resources.
• Complete ongoing 8-week Internal Programming.
• Support and attend events, tabling, and external programming.
