Case Manager

Department: Shared Living

Full Time

Location: Springfield, MA

Schedule: Monday-Friday, 9a-5p

Base Salary: $48,000–$50,000/year (based on experience, education, and workload)

Position Description

Using Positive Behavior Support (PBS) strategies, all DBIS staff are professional team members responsible for supporting each individual’s right to live a safe, enjoyable, meaningful life at home and in the community. As cooperative team members, we support environments that foster creative opportunities for the changing needs and desires of the people we serve.

Key Responsibilities
  • Assist with the start-up of the site and day‑to‑day operations.
  • Oversee Shared Living sites to ensure compliance with ServiceNet and state‑mandated health and safety standards.
  • Ensure the overall operation of the site(s) includes licensing regulations.
  • Oversee and ensure the health care, welfare and safety of the individuals we serve, including nutrition, dress, sleep, medical and dental services, social services and emergency services as needed.
  • Maintain appropriate daily activity and recreational programs for individuals.
  • Oversee individual money management needs.
  • Respond to crisis via the 24‑hour rotating on‑call system and handle emergency situations promptly.
  • Enforce division regulations and policies, including confidentiality.
  • Make decisions in crisis situations in the absence of the Director of Operations, with immediate notification to the supervisor or designee.
  • Communicate professionally with parents/guardians, hospitals, doctors, funders and community personnel.
  • Coach, train, supervise, mentor and evaluate Shared Living Providers and relief staff.
  • Attend relevant meetings and, when necessary, call additional meetings.
  • Schedule and attend at least one monthly site visit and document outcomes.
  • Attend regular meetings with the Director of Operations to coordinate business and supervision.
  • Arrange and provide assistance to individuals when Shared Living Providers need it.
  • Maintain a positive work atmosphere and collaborate effectively with individuals, providers, co‑workers and supervisors.
  • Serve as a team builder, supporter, and mentor with integrity and loyalty, dedicated to the organization's goals and mission.
  • Maintain strict confidentiality of all organizational and client information and adhere to federal and state regulations.
Minimum Qualifications
  • Bachelor’s degree preferred in a related field or three years of full‑time experience working with people who live with developmental delays and/or brain injuries.
  • Experience working with the DBIS population.
  • CPR and first aid certification required (or willingness to obtain prior to or upon hire).
  • MAP certification required (or willingness to obtain prior to or upon hire).
  • Human Rights Officer, Fire Safety Officer and PBS training as required per program.
