Program Manager – Case Manager for Minors and Non-Minor Dependents, Transitional Housing Program (THP)

Salary & Supervision

Salary: $24-$27 per hour

Supervisor: Program Manager (no direct reports)

Job Summary

The Case Manager for Minors and Non-Minor Dependents in the Transitional Housing Program (THP) provides direct support and intensive case management services to Transitional Aged Youth (TAY) ages 16 to 21 years, helping them meet individualized goals while connecting them with community resources. The average case load is 12 youth. The position is collaborative and coordinates care on an interdisciplinary team.

Responsibilities
  • Build and maintain therapeutic relationships with youth; implement supervision, authority, discipline, and foster engagement in a healthy home environment.
  • Conduct weekly individual contact with each youth: discuss goals, track progress, understand medications, help with prescription refills, and connect youth with community resources and external supports.
  • Support development of independent living skills and help youth achieve goals outlined in the Transition Independent Living Plan (TILP) and the Appraisal Needs and Service Plan.
  • Assist youth in identifying and pursuing employment, education, and housing goals; establish and monitor client budgets, submit purchase requests, and coordinate transportation as needed.
  • Participate in interdisciplinary team meetings, including Child Family Team meetings, TDMs, emancipation conferences, and maintain knowledge of the THP Program Statement.
  • Complete compliance and reporting tasks: Medi-Cal paperwork, client database entry within 2 days of information change, and maintain up‑to‑date records and documentation.
  • Maintain knowledge of CCL Regulations, ensure continuous compliance, and conduct regular audits of client homes where applicable.
  • Meet productivity standards: accurate, billable case notes per Medi-Cal requirements; achieve 75% youth graduation; complete assessments and documentation for all clients.
  • Serve as community liaison and agency representative; interface with County Child Welfare and Probation staff on case coordination.
  • Cover shifts as required and on an emergency basis.
Qualifications – Required
  • Bachelor’s Degree in Marriage, Family & Child Counseling, Child Psychology, Child Development, Counseling Psychology, Social Psychology, Clinical Psychology, Educational Psychology, Education with an emphasis on Counseling, Social Work or Social Welfare — areas that include counseling or psychotherapy content required for licensure as a Licensed Professional Clinical Counselor.
  • OR a Bachelor’s Degree in Human Services or Behavioral Science AND two (2) years of experience in a public or private child welfare social services setting, with specific experience working with TAY 16–24 years of age.
  • OR CCL approval as an Excepted Case Manager prior to start date.
  • Experience working with the TAY population.
  • Knowledge of Microsoft Office applications.
  • Demonstrated skills in case work, group work, and case coordination.
  • Understanding of transformational care planning and targeted case management principles of service delivery.
  • Excellent interpersonal skills (verbal and written) with demonstrated leadership in facilitating and linking services among a diverse variety of mental health programs.
  • Ability to maintain a positive perspective in the treatment and care of emotionally disturbed youth and young adults.
Qualifications – Preferred
  • Master’s Degree in any of the areas listed above.
  • Two (2) years’ experience working with at‑risk youth or adolescent children in a residential group home setting.
  • Bi‑Lingual in Spanish and English.
