Job summary

Stars Behavioral Health Group is seeking a Case Manager in Fresno, CA, for the Transitional Age Youth FSP program. The role involves delivering case management services at the office, in the community, and at clients' homes, with supervision for BBS hours where applicable.

Qualifications

• Associate degree in a social service field plus two years of mental/behavioral health experience
• Bachelor's degree in a social science field plus six months of experience (preferred). Internship experience may substitute for part of required work history.

Responsibilities

• Provide services at the office, in the community, and in clients’ homes.
• Deliver case management services to clients and families through assessment and collaboration with the treatment team.
• Attend treatment team meetings to ensure client needs are met and recommendations are followed.
• Document all services promptly and accurately in the Electronic Medical Record.
• Assist the primary therapist with initial and ongoing assessments of the client and family to identify strengths and needs.
• Monitor client progress toward treatment plan goals and inform the primary therapist of progress.

• Division/Program: Transitional Age Youth Full‑Service Partnership (TAY FSP)
• Starting Compensation: 22.00 - 28.00 USD per hour
• Working Location: Fresno, CA
• Working Hours/Shift: Mondays - Thursdays 10‑hour AM shift; Off Fridays. Hours approximate and may include flexibility as needed to meet client needs.

Qualifications

• Education: Associate degree in a social service field plus two (2) years of experience in a mental/behavioral health setting, or bachelor's degree in a social science field plus six (6) months of experience (preferred). Internship experience may substitute for part of required work history.
• Licensure & Compliance: Valid California Driver’s License and eligibility to meet agency driving criteria.

• Provides housing, education, vocational support, individual and family counseling, and medication management and support services.
