Job summary

Counseling and Research Associates, Inc. is seeking a Case Manager to provide case management and behavioral guidance for program youth in California. You will handle linkage services for youth and families, build skills for rehab services, and work designated shifts under supervision.

Travel to diverse environments may be required. The role focuses on developing measurable objectives, documenting contacts, and coordinating with parents, guardians, probation officers, CSWs, and school staff.

Qualifications

  • Bachelor's degree related to psychology, social work or related field.
  • Valid California Driver's License with good driving record.

Responsibilities

  • CASE MANAGEMENT: Provides general case management and skill building for client's and their families in accordance with the program policies.
  • OBJECTIVES: Develops measurable case management and individual rehab objectives for clients as outlined in the Client Care Plan and monitors progress toward goals.
  • PRODUCTIVITY: Complete thorough and timely documentation of all contacts with clients and maintain productivity standards.
  • MEETINGS: Attends staff meetings, in-service training and conferences as required.
  • CONSULTATION: Regularly consults with minor's parent/guardian, probation officer or CSW, and school staff.
  • TREATMENT TEAM: Meets with treatment team discussing client/program issues and coordinates services.
  • CONFIDENTIALITY: Maintains strict confidentiality over all records.
  • RESPECT: Demonstrates respect for individuals and cultures.
  • CRISIS: Ensures safety of youth and provides direct crisis intervention.
  • CLIENT INTEREST: Stimulates client interests in clinical offerings and maintains contact with client and family.
  • BOUNDARIES: Maintains professional boundaries with clients and families.
  • TRANSLATION: Provides translation services as needed.
  • STRENGTH BASED: Identifies and builds on client strengths.
  • TRAVEL: Travels independently to various environments to provide services.
  • OTHER: Performs other duties as assigned by supervisor.

Job description

A Case Manager provides case management and behavioral guidance for program youth. Persons employed as case managers are responsible for behavioral case management and linkage services for the youth and families being served as well as skill building for individual rehab services. The position involves working designated shifts under the direct supervision of the Clinical Supervisor or Program Director.

DUTIES
  • CASE MANAGEMENT: Provides general case management and skill building for client's and their and families in accordance with the policies of the program and referring/regulatory agencies (DMH, DCFS, Probation, schools).
  • OBJECTIVES: Develops measurable case management and individual rehab objectives for clients as outlined in the Client Care Plan and monitors progress toward those goals.
  • PRODUCTIVITY: Completes thorough and timely documentation of all contacts with clients and maintains productivity standards set forth by the agency Tier System and internal policies.
  • MEETINGS: Attends all staff meetings, in-service training and conferences as required and scheduled.
  • CONSULTATION: Regularly consults with minor's parent or guardian, probation officer or CSW, and school staff.
  • TREATMENT TEAM: Meets regularly with other members of the treatment team concerning client and program issues and coordinates needed services and linkages.
  • CONFIDENTIALITY: Maintains strict confidentiality over all records and information relative to minors.
  • RESPECT: Demonstrates respect for the intrinsic value of each person, their individual differences and culture.
  • CRISIS: Ensures the safety of youth. Provides direct crisis intervention and therapeutic guidance.
  • CLIENT INTEREST: Stimulates client's interests in various clinical offerings. Monitors progress and maintains close contact with client and family.
  • BOUNDARIES: Establishes positive, respectful and nurturing relationships with clients and families within professional boundaries and in accordance with agency policies and procedures.
  • TRANSLATION: Provides translation services as needed.
  • STRENGTH BASED: Is capable of accurately identifying and building on the strengths of clients to better reach their potential.
  • TRAVEL: Travels independently to various environments (buildings with stairs, various communities, schools, etc.) to provide services as needed.
  • OTHER: Performs other duties as assigned by supervisor.
QUALIFICATIONS
  • Bachelor's degree related to the fields of psychology, social work or other majors defined by the Department of Mental Health as a related degree.
  • Valid California Drivers License, good driving record, and must be able to drive to fulfill the duties of the position.
