Job summary

Hines is seeking a Case Manager with a nursing background to develop care plans and advocate for patients' health needs in a full-time role. You will use your clinical nursing experience to coordinate services and support quality care in collaboration with the care team.

The position offers a Monday–Friday schedule with no weekends or nights, and a hybrid option after orientation. A competitive benefits package and 401(k) match are provided.

Qualifications

• RN license with unrestricted practice in IA or reciprocity.
• Successful completion of case management orientation program.
• Minimum 2 years full-time acute inpatient or specialty area.
• Excellent communication skills.
• Typing speed 35 WPM; case management experience helpful but not required.
• Previous case management or insurance industry experience helpful but not required.

Responsibilities

• Develop care plans using clinical nursing experience to meet patient needs.
• Be a patient advocate and coordinate care with providers.
• Support specialty case management areas (Oncology, Chronic Condition, Behavioral Health) as needed.
• This is a full-time position with a Monday–Friday schedule.
• Hybrid work option after orientation.
• No weekends or holidays.
