Qualifications

• Minimum of 2 years full-time Acute inpatient setting or specialty area.
• Excellent communication skills.
• Previous case management or insurance experience helps but not required.

Responsibilities

• Develop care plans to meet patient health care needs.
• Advocate for patients and coordinate services.
• Maintain timely documentation and coordinate with care teams.
• Participate in orientation and transition to hybrid work arrangement after onboarding.
