Qualifications

• RN licensed/certified with unrestricted license in the state or reciprocation state.
• Minimum of 2 years full-time acute inpatient setting or specialty area (ICU, ER, Oncology, NICU, Acute Rehab).
• Excellent communication skills; Basic typing/computer knowledge (minimum 35 WPM).
• Previous case management experience helpful but not required.

Responsibilities

• Develop a plan for meeting the patient’s health care needs using your nursing experience.
• Advocate for patients and coordinate care with healthcare providers.
• Becomes a patient’s care coordinator, contributing to ongoing health care knowledge.
