Job summary

The position is full‑time, Monday–Friday, in-office with a potential hybrid option after orientation. Licensing must be unrestricted in the state, with orientation completing before any hybrid work is

Qualifications

Must be an RN with an unrestricted license in the state of the office or licensure reciprocity with that state.
Successful completion of case management orientation program
Minimum of 2 years full-time acute inpatient setting or specialty area (ICU, ER, Oncology, NICU, Acute Rehab)
Excellent communication skills
Basic typing/computer knowledge with a minimum speed of 35 WPM
Previous case management or insurance industry experience helpful but not required.

Responsibilities

Develop and implement care plans based on clinical nursing experience to meet patient health care needs.
Act as patient advocate and coordinate care across providers.
This is a full-time, Monday–Friday role with no weekends or nights; after orientation a hybrid option may be available.
Be a supportive, collaborative team member in a patient-focused environment.
