Job summary

Holisticare Hospice in Philadelphia offers a Full-Time Hospice RN Case Manager (RNCM) role delivering hands-on nursing care, coordinating services, and guiding patients and families through the hospice journey.

As part of an interdisciplinary team, you’ll assess eligibility with the Clinical Director, educate families, and provide compassionate, evidence-based care in the community. Requirements include RN license, 1 year acute care, EMR proficiency, and a valid driver's license.

Qualifications

• Hold a current, unencumbered RN license.
• Minimum 1 year experience as an RN in medical-surgical/acute care; hospice a plus.
• Proficiency with EMR and computer programs.
• CPR certification if required by state.
• Valid driver’s license and reliable transportation.

Responsibilities

• Respond to clinical referrals with care and expertise.
• Collaborate with Clinical Director to assess client eligibility for hospice services.
• Provide direct nursing care and coordinate services to support comfort and quality of life.
• Deliver nursing care grounded in evidence and compassion.
• Educate patients and families to participate in personalized care planning.
• Share call duties and be available for compassionate moments when it matters.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Minimum one (1) year experience as an RN in a medical surgical/acute care setting. Hospice exp a plus!
• Must be computer proficient in typing and various programs, including background in EMR.
• Possess and maintains current CPR certification if required by state.
• Current driver's license, vehicle insurance, and access to dependable transportation.

$95,000 - $97,000 per year (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
