The Care Team Hospice is seeking a compassionate and patient-focused Hospice RN Case Manager to join our team, delivering exceptional end-of-life care wherever they call home.

You will coordinate patient care, manage individualized plans of care, and collaborate with physicians, caregivers, and the interdisciplinary team to ensure comfort, dignity, and quality of life for patients and families.

Qualifications

• Active RN License required
• Hospice nursing experience preferred
• Experience with Homecare Homebase (HCHB) preferred
• Strong assessment, communication, and care coordination skills

Responsabilités

• Provide skilled nursing care, assessments, and symptom management in accordance with hospice standards, physician orders, and individualized plans of care.
• Complete scheduled visits, admissions, and clinical interventions while maintaining accurate and timely documentation in Homecare Homebase (HCHB).
• Respond promptly to patient, family, and caregiver needs, including after-hours and on-call situations, to ensure exceptional patient-centered care.
• Collaborate with physicians, caregivers, and interdisciplinary team members to coordinate care and support positive patient outcomes.
• Educate patients, families, and caregivers on symptom management, disease progression, medications, and end-of-life care while providing compassionate support.

Schedule & Work-Life Balance

• Monday–Friday, 8:00 AM – 4:30 PM
• On-call opportunities available with additional compensation
• Some locations offer dedicated on-call teams, resulting in backup coverage only
