The R3 Case Manager provides comprehensive, participant-centered case management to individuals enrolled in Proactive Community Services' Restore, Reinvest, and Renew (R3) Program. The position assesses participant needs, develops individualized service plans, coordinates referrals and supportive services, monitors progress, and helps participants address barriers affecting employment, housing stability, economic self-sufficiency, and overall well-being. The Case Manager works closely with PCS staff, community organizations, workforce development providers, employers, landlords, and referral partners to support coordinated service delivery and participant retention.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Conduct participant intake, eligibility screening, comprehensive needs assessments, and enrollment activities.
- Develop individualized service and case management plans based on participant needs, goals, strengths, and identified barriers.
- Maintain regular contact with assigned participants and monitor progress toward established goals and service plan objectives.
- Provide case management through office visits, community-based meetings, participant home visits, telephone contacts, and other approved methods.
- Identify barriers related to housing, employment, transportation, education, financial stability, healthcare, behavioral health, substance use, and other social determinants of health.
- Connect participants to appropriate PCS programs and external community resources.
- Coordinate referrals for workforce development, vocational training, employment services, housing assistance, healthcare, behavioral health, benefits, transportation, and other supportive services.
- Assist eligible participants with accessing grant-supported housing stabilization resources, including rental arrears, utility assistance, and security deposit assistance.
- Coordinate with the Community Resource Navigator, Housing Navigator, Program Coordinator, and other program staff to promote continuity of services.
- Advocate for participants and assist them in navigating complex service systems.
- Follow up with participants and referral agencies to verify service connection, participation, and outcomes.
- Participate in multidisciplinary case reviews, staff meetings, and program quality-improvement activities.
- Maintain complete, accurate, timely, and confidential participant records, including case notes, assessments, service plans, referrals, and supporting documentation.
- Enter required participant and service information into PCS-approved data and case management systems.
- Track participant progress and outcomes required by PCS and the R3 grant.
- Assist with collecting documentation needed for grant reporting, monitoring, quality assurance, and compliance.
- Maintain participant confidentiality and comply with PCS policies and applicable state and federal requirements.
- Participate in community outreach, partner meetings, and other grant-related activities as assigned.
- Perform other program-related duties consistent with the R3 grant and PCS operations.
KNOWLEDGE, SKILLS, AND ABILITIES
- Knowledge of case management and community-based human services.
- Understanding of barriers affecting economically disadvantaged and underserved populations.
- Knowledge of housing, workforce development, healthcare, behavioral health, benefits, and community resources.
- Ability to conduct comprehensive assessments and develop individualized service plans.
- Strong participant engagement, advocacy, problem-solving, crisis-response, and follow-up skills.
- Ability to establish professional relationships with participants, employers, landlords, providers, and community partners.
- Strong written documentation, time-management, and organizational skills.
- Ability to manage multiple participants, deadlines, referrals, and follow-up activities.
- Ability to work effectively with individuals from diverse cultural and socioeconomic backgrounds.
- Proficiency with Microsoft Office and electronic case management/data systems.
- Ability and willingness to provide services in community-based settings throughout South Suburban Cook County.
MINIMUM QUALIFICATIONS
- Bachelor's degree in Social Work, Psychology, Human Services, Sociology, Criminal Justice, Public Health, or a related field preferred.
- Relevant case management or community-based human services experience may be considered in combination with education.
- Minimum of two years of experience providing case management, social services, workforce development, housing navigation, or related services preferred.
- Experience working with underserved, justice-impacted, economically disadvantaged, or high-need populations strongly preferred.
- Valid driver's license, reliable transportation, and ability to travel throughout the program service area as required.
The Case Manager is expected to maintain timely and accurate documentation, actively engage assigned participants, complete required follow-up, facilitate successful referrals, support participant retention, and contribute to achievement of R3 program performance outcomes.
