Job summary

Proactive Community Services, Inc. seeks a dedicated R3 Case Manager to deliver participant-centered case management within the Restore, Reinvest, and Renew program.

The role focuses on assessing needs, creating tailored service plans, coordinating referrals, and monitoring progress to address barriers to housing, employment, and well-being. You will collaborate with PCS staff, community partners, and providers to ensure service continuity and participant retention while maintaining confidential

Qualifications

• Bachelor's degree in Social Work or related field preferred.
• Minimum two years of case management or related experience preferred.
• Experience with underserved populations preferred.
• Valid driver's license and ability to travel throughout the service area.

Responsibilities

• Conduct intake, eligibility screening, and needs assessments.
• Develop individualized service and case management plans.
• Maintain regular contact and monitor progress toward goals.
• Provide case management through office visits and community-based meetings.
• Identify barriers related to housing, employment, transportation, and health.
• Coordinate referrals for workforce development, housing, healthcare, and other services.
• Advocate for participants and ensure confidentiality and compliance.
• Enter required data into PCS-approved systems and participate in reviews.
