Job summary

Apis Services, Inc. is seeking a compassionate Case Manager to help individuals navigate behavioral health, substance use, and developmental disability services in community settings. You will connect participants to resources, coordinate care, and support recovery goals.

The role requires building partnerships with providers, families, and clinical staff while maintaining person-centered, recovery-oriented services. Travel within the service area and driving may be required.

Qualifications

  • Bachelor's degree in a related field.
  • 1 year of case management experience preferred.
  • 2 years of experience in behavioral health or related services preferred.

Responsibilities

  • Participate as a member of a multidisciplinary treatment team and assist in developing individualized recovery plans.
  • Meet participants in community settings and connect them to behavioral health, housing, employment, and community services.
  • Advocate for participant needs, coordinate resources, and monitor progress toward recovery goals.
  • Ensure compliant documentation of assessments, plans, progress notes, and reports.

This role is ideal for someone who enjoys working directly with people, building community partnerships, and making a meaningful difference in the lives of individuals receiving behavioral health, substance use, or developmental disability services.

Position Summary

The Case Manager provides community-based case management services designed to help participants access the resources and supports necessary to meet their individualized needs.

This position works directly with participants in community settings, serving as an advocate, resource coordinator, and member of a multidisciplinary treatment team. The Case Manager assists with the implementation of treatment and recovery plans, identifies service gaps, and connects individuals to community-based supports that promote long-term success and independence.

Essential Duties and Responsibilities
Team-Based Planning & Collaboration
  • Participate as a member of a multidisciplinary treatment team.
  • Assist in the development and ongoing review of individualized recovery plans.
  • Collaborate with participants, families, clinical staff, medical providers, and community partners.
  • Support a holistic, strengths-based approach to service delivery.
  • Advocate for participant needs and self-determination.
Community-Based Case Management
  • Meet with participants in community settings, homes, and other service locations.
  • Assess participant needs and identify appropriate resources and supports.
  • Connect individuals to behavioral health, healthcare, housing, employment, educational, vocational, and community services.
  • Assist participants in navigating complex service systems.
  • Monitor progress toward recovery and treatment goals.
  • Help address barriers to successful community living.
Resource Coordination & Advocacy
  • Develop and maintain relationships with community agencies and providers.
  • Identify service gaps and barriers impacting participants.
  • Advocate for participant access to needed services and supports.
  • Assist participants in obtaining benefits, resources, and community services.
Documentation & Compliance
  • Complete required assessments, service plans, progress notes, and reports.
  • Maintain accurate and timely documentation in accordance with agency, funding, licensing, and accreditation standards.
  • Ensure confidentiality and compliance with all applicable regulations and policies.
  • Complete other administrative duties as assigned.
Qualifications
Required Education
  • Bachelor's Degree in Human Services, Social Work, Psychology, Sociology, Counseling, or a related field.
Preferred Experience
  • One (1) year of Case Management experience.
  • Two (2) years of experience supporting individuals in:
  • Behavioral Health
  • Substance Use Recovery Services
  • Intellectual and Developmental Disabilities
  • Human Services Programs
Additional Requirements
  • Valid Driver's License
  • Ability to travel throughout the service area
  • Intermediate computer skills

Working Conditions

This position is primarily community-based and may involve:

  • Travel throughout the program service area
  • Home visits and community appointments
  • Transportation of participants to appointments and services when required
  • Exposure to varying environmental and weather conditions

Potential exposure may include:

  • Dust, fumes, and outdoor conditions
  • Extreme heat or cold
  • Loud noise environments
  • Community and public settings
Physical Requirements

The employee must be able to:

  • Stand, sit, and walk for extended periods
  • Climb stairs and navigate community environments
  • Reach, bend, stoop, kneel, crouch, and crawl when necessary
  • Push and pull objects
  • Perform repetitive motions such as typing and documentation
  • Lift and move up to 50 pounds occasionally
  • Drive regularly throughout the service area
