Job summary

Clarksville Rehabilitation Hospital is seeking a Case Manager to coordinate patient progress through the continuum of care in collaboration with physicians. You will manage clinical and financial coordination of treatment plans to ensure timely, cost-effective, individualized service delivery for diverse rehabilitation patients.

Qualifications

• Current state licensure as a Registered Nurse or Licensed Social Worker.
• 2 years experience in a hospital setting preferred.
• Certification in case management preferred.

Responsibilities

• Provide program management for each patient to ensure progression through the continuum of care in collaboration with the physician.
• Monitor and coordinate the treatment plan for assigned patients to ensure timely, cost-effective, individualized service delivery.
• Work with rehabilitation patients with various disabilities including spinal cord injury, brain injury, stroke, amputation, neurologic disorders, orthopedic conditions, and arthritis.
• Coordinate length of stay management within CMS guidelines and 60% compliance threshold.
• Provide community resources including vocational rehabilitation resources.

Job description

Clarksville Rehabilitation Hospital is a brand new startup with 40 private patient suites and set to take its first patient October 2026.

Position Summary:

The Case Manager, in collaboration with the physician, providesindividual program management for each patient toensure the patient’s progression through the continuum of care in a manner that achieves the desired clinical andfinancial outcomes. Monitors and manages clinical and financial coordination of treatmentplanofassigned patientsto ensuretimely, cost-effective, individualized service delivery. Works with rehabilitation patients with variousdisabilities including, but not limitedto:spinal cord injury, brain injury, cerebrovascular accident, amputation,neurologic disorders, orthopedicconditions, and arthritis. Coordinates length of stay management within Medicare(CMS) guidelines and 60% compliance threshold. Provides community resources as appropriate including vocational rehabilitation resources.

Pay:Rate of pay is based on years of experience and qualifications.

Minimum Qualifications:

• Current state licensure as a Registered Nurse or Licensed Social Worker.

Desired Qualifications:

• 2 years experience in a hospital setting preferred.
• Certification in case management preferred.
