Job summary

Florida Rehabilitation Hospital at Tampa is seeking a skilled Case Manager to coordinate patient care in collaboration with physicians, ensuring progression through the continuum of care and adherence to CMS guidelines.

You will monitor clinical and financial aspects of treatment plans for patients with spinal cord injury, brain injury, stroke, amputations, and other conditions, aiming for timely, cost-effective, individualized service delivery.

Qualifications

• Current state licensure as a Registered Nurse or Licensed Social Worker.
• MSW without licensure will be considered.
• Minimum of 3 years healthcare and clinical experience in a hospital setting.

Job description

Florida Rehabilitation Hospital at Tampa located at Woodstork and Temple Terrace. Join our team to work in a beautiful new hospital with 60 private patient rooms and 2 rehab therapy gyms with cutting-edge technology.

The Case Manager, in collaboration with the physician, provides individual program management for each patient to ensure the patient’s progression through the continuum of care in a manner that achieves the desired clinical and financial outcomes. Monitors and manages clinical and financial coordination of treatment plan of assigned patients to ensure timely, cost-effective, individualized service delivery. Works with rehabilitation patients with various disabilities including, but not limited to: spinal cord injury, brain injury, cerebrovascular accident, amputation, neurologic disorders, orthopedic conditions, and arthritis. Coordinates length of stay management within Medicare (CMS) guidelines and 60% compliance threshold. Provides community resources as appropriate including vocational rehabilitation resources.

Pay: Rate of pay is based on years of experience and qualifications.

Minimum Qualifications:

• Current state licensure as a Registered Nurse, Licensed Social Worker,
• MSW without licensure will be considered
• Minimum of 3 years healthcare and clinical experience in a hospital setting

Desired Qualifications:

• 1 years experience in a hospital setting preferred
• Certification in case management preferred.
