Job description

The HCBS Coordinator plays a key role in supporting individuals with intellectual and developmental disabilities to live meaningful lives in the community. This position involves coordinating services funded by the Office for People With Developmental Disabilities (OPWDD), ensuring compliance with regulatory standards, and promoting person-centered practices. The ideal candidate is compassionate, organized, and committed to advocacy, dignity, and inclusion.

Key Responsibilities:

  • Coordinate a range of OPWDD-funded HCBS services including day habilitation, residential supports, respite, and clinical interventions.
  • Develop, implement, and monitor Person-Centered Service Plans (PCSPs) in collaboration with individuals, families, and service providers.
  • Conduct routine home visits, site reviews, and annual assessments to ensure service quality and alignment with individual goals.
  • Maintain accurate and timely documentation in accordance with OPWDD guidelines and agency protocols.
  • Serve as a liaison between individuals, families, providers, and regional OPWDD representatives to facilitate seamless care coordination.
  • Identify service needs and advocate for appropriate supports, including crisis intervention and linkage to community resources.
  • Ensure compliance with Medicaid Waiver requirements and support audit readiness through thorough documentation and case management.

Requirements:

  • Strong organizational skills with the ability to manage multiple caseloads while maintaining accuracy.
  • Excellent interpersonal, verbal, and written communication skills.
  • Ability to work independently and collaboratively as part of an interdisciplinary team.
  • Knowledge of OPWDD regulations, HCBS Waiver services, and person-centered planning principles.

Qualifications:

  • Associate’s degree required; Bachelor’s degree in social work, psychology, human services, or a related field preferred.
  • Minimum 1–2 years of experience working with individuals with developmental disabilities, preferably within the OPWDD or Medicaid service system.
  • Proficiency in Microsoft Office and familiarity with electronic health records (EHR).
  • CPR and First Aid certification preferred.
  • Bilingual candidates are strongly encouraged to apply.
