Job summary

St. Joseph Center in Los Angeles, CA is hiring a Case Manager to work with the Office of Diversion and Re-entry.

You will provide case management and supportive services to meet the needs of each individual, link clients to housing programs, health services, and benefits, and maintain organized files in HMIS. The role requires at least 2+ years of experience with vulnerable populations, a Bachelor’s degree in a related field, and proficiency with MS Office.

Qualifications

• 2+ years of experience providing services to highly vulnerable populations.
• Bachelor’s degree in Psychology, Social Work, Human Services or related field.
• Proficiency in MS Office Suite.
• Valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions.
• Travel requirements: Access to reliable transportation.

Responsibilities

• Develop and maintain strong ties to the community, law enforcement, and other homeless service providers.
• Link clients to appropriate bridge/interim housing programs and assist them in completing required paperwork in order to enroll them in permanent housing-related programs.
• Provide case management and supportive services to meet the needs of each individual.
• Build trust and rapport with individuals experiencing homelessness and assess using the Vulnerability Index (VI) SPDAT.
• Link clients to physical health, mental health, and other supportive services; assist enrollment in mainstream benefits and identification.
• Maintain organized client files and enter data into HMIS.
• Provide bus tokens, passes, taxi vouchers, or direct transportation as needed.
• Participate in case conferencing and community meetings.
• Coordinate with other outreach teams in joint outreach efforts.
