Job summary

Lutheran Community Services NW is a nonprofit serving Washington, Oregon and Idaho. The role supports health, justice and hope by outreaching to communities, providing case management, and guiding clients through resources and benefits.

Position requires fluency in English and Ukrainian or Russian; duties include outreach, relationship building, and coordination with partner agencies in diverse settings.

Responsibilities

  • Outreach communities, individuals, and businesses to inform them of program opportunities and services.
  • Develop and maintain relationships with communities to reduce barriers between clients and institutions.
  • Provide culturally and linguistically appropriate case management services and assist with language needs.
  • Accept client referrals and manage response times per program while addressing needs.
  • Provide Resource Navigation to connect clients with workshops, housing, benefits and services.
  • Maintain contact, follow up, and discharge of clients as outlined by program guidance.

HOW YOU WILL MAKE AN IMPACT:

**Please note this position requires demonstrated fluency, both spoken and written, in English and one of the following languages: Ukrainian and/or Russian**

  • Outreach communities, individualentities,and businesses to inform them of program and program opportunities. Convey the purposes and services of the program being represented to the user population and the impact that program or service would have on their health and wellbeing.
  • Develop,workeffectively,andmaintainrelationships with communities to reduce cultural and socio-economic barriers between clients and institutions.
  • Provide culturally and linguisticallyappropriate casemanagement services to qualified program participants.Communicate information to clients and providers clearly and succinctly, both written andverbally.Assistwith language and interpretation needs as necessary
  • Accept client referrals as directed by each program and manage response times asrequiredper program.Assess and address needs as presented anddeterminedby individual program assessment requirements.
  • ProvideResource Navigation:Assistindividuals in obtainingappropriate resourcesand connect to applicablecommunity entitiesthat may include workshops/educational resources, respite care services,housing,referrals to state and federal benefits, nutrition programs, family caregiver programs, kinship care,food banks,Medicaidand/or otherinsuranceservices available.
  • Maintain contact, followup,anddischarge ofclients as outlined in individual programs.
  • Ensureappropriate documentationand gather outcome data under the direction of the Program Supervisor. Document in applicable program system as needed
HOW YOU WILL BE A GREAT FIT:
  • 1-2years of related experienceor l ived experience that enhances relatability to clients being served
  • HS diploma or equivalent-Preferredbachelor'sdegreein related field. However,substantial relevant experience will be considered in place of degree
  • Demonstrated fluency, both spoken and written, in English and one of the following languages: Ukrainian, Russian, Korean, Japanese, Hindi, Chinese/Mandarin
  • Intermediatemastery of business applications and productivity suites
  • Demonstrates resilience, awareness and self-regulation in navigating challenges,conflictand obstacles/barriers.
  • Thrives in a diverse and multicultural environment; works with cross-functional teams to develop and implement strategies to enhance end-user experiences throughout the service lifecycle
  • Demonstrates exceptional commitment to serving and advocating for diverse and marginalized populations
  • Successfully completes tasks ina moderatelynoisy environment
  • Interacts with others and performs tasks in-person andthrough the use oftechnology, withlongperiodsof stationary activity andminimalperiods of increased physical activityincluding and not limited tostanding at event tables indoors and outdoors.
  • Moves equipment/materials weighing up to15pounds
  • Travels between worksitesoccasionally
  • Works in a variety of settings, including and not limited to LCSNW office locations, partner agency,community-based settings, and faith-based settings such as churches
  • Meets LCSNW criminal background clearance requirements and other job-relevant clearances as required by the State or program
