Job summary

Lutheran Community Services NW in Portland, OR is hiring a Housing Stability Case Manager to help adults secure and maintain safe housing through case management, financial assistance, and housing navigation.

You will work with partners, landlords, shelters, and health systems to deliver trauma-informed, equitable services, using HMIS or similar databases; associate degree or equivalent experience preferred.

Qualifications

  • Associate degree in social work, human services, psychology, counseling, social sciences, or related field.
  • Two years of relevant experience in case management, housing navigation, homeless services, or related supports.
  • Reliable transportation for business purposes and ability to travel across Multnomah County.

Responsibilities

  • Provide case management services to support housing placement and stability.
  • Assess client needs and develop individualized service plans.
  • Assist clients with housing searches, applications, and documentation.
  • Coordinate housing-related financial assistance and community resources.
  • Support clients in accessing benefits, healthcare, and other services.
  • Collaborate with landlords, shelters, and community partners to promote housing outcomes.
  • Maintain accurate documentation in HMIS and other required databases.
  • Deliver services using trauma-informed, culturally responsive, client-centered practices.

HOW YOU WILL MAKE AN IMPACT:

The Housing Stability Case Manager provides trauma-informed, culturally responsive case management and housing stabilization services to adults experiencing or at risk of homelessness. Working collaboratively with community partners, landlords, shelter providers, and healthcaresystems, this position helps individuals obtain andmaintainsafe, stable housing through housing navigation, financialassistancecoordination, landlord engagement, and resource connection. In this role, you willutilizeyourexperience serving individuals experiencing homelessness, administering housing-related financialassistance, and using HMIS,WellSky, or similar case management databases. Knowledge of Multnomah County's homeless services system and community resources is preferred, along with a commitment to person-centered,equitable service delivery

HOW YOU WILL BE A GREAT FIT:
  • Strong written and verbal communication skill
  • Ability to work effectively with individuals experiencing homelessness, behavioral health challenges, substance use disorders, trauma, and other barriers to stability.
  • Knowledge of housing resources, landlord-tenant systems, public benefits, and community-based services.
  • Ability to build positive working relationships with clients, landlords, community partners, and service providers.
  • Strong organizational skills and attention to detail.
  • Ability to prioritize tasks and manage competing demands.
  • Proficiency with electronic databases and documentation systems
  • Ability to work independently in community-based settings.
  • Demonstrated commitment to equity, inclusion, and culturally responsive service delivery.
  • Ability tomaintainconfidentiality and professional boundaries.
  • Associate degree in social work, human services, psychology, counseling, social sciences, or a related field; OR
  • Two years of relevant experience providing case management, housing navigation, homeless services, behavioral health services, employment services, refugee resettlement services, or other human servicessupports
  • Reliable transportation for business purposes, such as between work locations and community settings as this position drives clientsin personal and/or agency rental.
  • Ability to travel throughout Multnomah County to meet with participants and community partners.
