HOW YOU WILL MAKE AN IMPACT:
The Housing Stability Case Manager provides trauma-informed, culturally responsive case management and housing stabilization services to adults experiencing or at risk of homelessness. Working collaboratively with community partners, landlords, shelter providers, and healthcare systems, this position helps individuals obtain and maintain safe, stable housing through housing navigation, financial assistance coordination, landlord engagement, and resource connection.
In this role, you will utilize your experience serving individuals experiencing homelessness, administering housing-related financial assistance , and using HMIS, WellSky , or similar case management databases. Knowledge of Multnomah County's homeless services system and community resources is preferred, along with a commitment to person-centered, equitable service delivery.
