Job summary

The Salvation Army in Oklahoma City is seeking a Case Manager to provide comprehensive, long-term case management for an assigned caseload within an established life management program. You will interview clients, assess needs, develop program plans, and advocate for services while ensuring compliance with funding requirements.

You will conduct eligibility interviews, coordinate referrals to external resources, and maintain detailed records.

Qualifications

• High school diploma or GED required.
• Two years’ experience in a social/public service environment.
• Valid State Driver License.

Responsibilities

• Interviews applicants to determine eligibility for program enrollment based on facility guidelines.
• Conduct needs assessments and refer applicants to appropriate services or agencies.
• Establish long-term program goals and counsel clients on modifying negative behaviors.
• Coordinate with external case management professionals to meet holistic client needs.
• Assist clients in obtaining income, benefits, and other supports.
• Maintain accurate case records, incident reports, and progress notes.
• Discharge planning for clients exiting the program and document outcomes.
• Cross-train to support other Social Services roles as needed.
• Attend community meetings to educate on the program and refer clients.
