Job summary

The Salvation Army in Austin, TX is seeking a Case Manager to deliver structured, long-term case management services for an assigned caseload within an established life management program.

You will tailor comprehensive program plans, mentor clients, and advocate to obtain services while ensuring compliance with funding requirements across housing, food, and social services. Strong documentation and collaboration with partners are essential.

Qualifications

• Bachelor's degree in Social Work, Behavioral Science, or related field.
• Three years of progressive case management experience in social services.
• Experience developing long-term action plans for recovery.
• Equivalent combination of training and experience accepted.

Responsibilities

• Establishes long-term program goals for eligible clients and explains them clearly.
• Outsources clients to additional counseling resources as needed.
• Meets regularly with clients to discuss progress and documents records.
• Completes discharge plans for clients exiting the program.
• Interacts with community agencies to coordinate assistance for clients.
