Community Mental Health Affiliates, Inc. (CMHA) is a private nonprofit provider serving Waterbury and surrounding areas. We seek a full-time Case Manager for our Greater Waterbury Care Center, 8:30am-4:30pm, Monday–Friday, located at 255 Bank St., Waterbury, CT 06051.

Qualifications

• Bachelor’s Degree in a mental health related field preferred or equivalent experience.
• Must have valid Connecticut driver’s license and a reliable vehicle.
• Will become certified in MANDT, First Aid, and CPR.
• Experience in Case Management or related field, preferably 1+ year.
• Proficient with Microsoft Office (Word, Outlook, Excel, PowerPoint) and EMR systems.

Responsibilities

• Maintains caseload and provides direct care, education, and support to clients.
• Develops and reviews individualized treatment plans with clients.
• Documents progress notes, plans, and required records per policy.
• Coordinates with treatment teams and external providers to ensure access to services.
• May provide transportation and support with daily living activities.
