Community Health Resources is seeking a dedicated Case Manager for the Eastern Region Service Center in Willimantic, CT. This full-time role supports medically indigent and Medicaid populations, helping clients access treatment, housing, employment, and education services.

The position emphasizes client engagement, collaboration with a network of partner agencies, crisis intervention when needed, and adherence to program standards. A bachelor’s degree in human services is preferred.

Qualifications

• Bachelor’s degree in human services preferred or equivalent work experience.
• At least two years’ experience in a mental health setting preferred.
• Valid driver’s license.

Responsibilities

• Uses appropriate therapeutic models to aid clients in understanding and managing their symptoms.
• Conducts program specific assessments and screens as required by program.
• Provides case management; coordinates with client’s supports and treatment teams; connects to prosocial activities; provides crisis intervention as needed.
• Demonstrates competency in age-related or specialty issues for the population served.
• Teaches and supports learning of skills including emotion regulation and age-appropriate activities.
• Documents all services and contacts per agency standards.
• Transports clients as required by program.
• Participates in staff meetings, supervision, training and other meetings as assigned.
• Additional duties as required.

ABOUT THE PROGRAM

Eastern Region Service Center (ERSC) is a regional case management project that is administered by Advanced Behavioral Health to assist clients who are medically indigent or eligible for or currently receiving Medicaid LIA.

CHR case managers work with the staff of 5 other agencies in the eastern portion of Connecticut to provide assessments and other case management services to clients with substance abuse and/or mental health issues.

Case Managers engage, encourage and assist clients in obtaining treatment and housing as well as employment, education and medical services.

ABOUT THE POSITION: Case Manager

Duties & Responsibilities

• Utilizes appropriate therapeutic models to aid clients in understanding and managing their symptoms in a variety of environments.
• Conducts program specific assessments and screens as required by program
• Provides case management. Works in close collaboration with the client’s natural supports and treatment teams. Connects to prosocial activities. Provides crisis intervention and de-escalation as needed.
• Demonstrates competency in age related or specialty issues and developmental needs for each population served. Demonstrates program specific target population competencies.
• Demonstrates the ability to teach and support the learning of a broad range of skills including those related to emotion regulation, age-appropriate activities and competencies and living in a home-like setting. Plan and actively participate in activities to support and increase skills and competencies.
• Documents all services and contacts with clients, collateral contacts and appointments in compliance with agency standards.
• Transports clients as required by program.
• Participates in staff meetings, supervision, training and other meetings as assigned.
• Additional duties as required.

QUALIFICATIONS

Education: Bachelor’s degree in human services preferred, or education plus equivalent work experience required.

Experience: At least two (2) years’ experience in a mental health setting preferred.

Licensure/Certification/Registration: Valid driver’s license.

$19.28/hr- $28.71/hr
