Job summary

Community Health Resources (CHR) in Willimantic, CT is seeking a Case Manager for the Oak Street program. You will coordinate services for adults with substance use disorders, perform assessments, and connect clients to outpatient treatment and community resources.

The ideal candidate has an Associate degree or higher, at least two years in a mental health setting, and a valid driver’s license. You will engage in crisis intervention, documentation, and collaborative teamwork to support clients’

Qualifications

• Associate's degree or higher.
• Bachelor's degree or equivalent experience preferred.
• Two years’ experience in a mental health setting preferred.
• Valid driver’s license is required.

Responsibilities

• Conduct program assessments and screens.
• Provide case management and crisis intervention.
• Collaborate with treatment teams and natural supports.
• Document all services and contacts in agency records.
• Transport clients as required.
• Participate in staff meetings and trainings.
• Support clients in building skills and independence.
