Job summary

Terros Health in Phoenix seeks a Case Manager to join our interdisciplinary clinical team working with individuals diagnosed with Serious Mental Illness or General Mental Health under Court Ordered Treatment at Desert Cove Recovery Health Center.

This full-time role offers comprehensive benefits, 40 hours per week, Monday through Friday, 8:00 am to 5:00 pm, located at 10844 N. 23rd Ave., Suite #200, Phoenix, AZ 85029. Training and growth opportunities abound in behavioral health.

Qualifications

High-quality communication and coordination with clients, families and providers.
Proven ability to complete assessments and individualized service plans.
Willingness to pursue ongoing education and training in behavioral health.

Responsibilities

Assists clients with identifying treatment and recovery goals and provides supportive services to help clients reach their goals.
Completes assessments, crisis plans and individualized service plans.
Works collaboratively with the clinical team to engage, educate, communicate and coordinate care with member, their family, behavioral health providers, medical and dental providers, community resources and others in ensuring that all services prescribed in the individualized service plan are implemented.
Conducts outreach and follow-up of services including crisis interaction, missed appointments, and hospital discharges to ensure resources are available.
Performs all case management functions including assessment and service planning; identifying the need for further or specialty evaluations.
Pursues best practice outcomes for persons with mental illness including continuing education, employment, independent housing and community tenure.
