Job summary

Terros Health is seeking a Case Manager to join our interdisciplinary clinical team in Phoenix, AZ. The role focuses on individuals with Serious Mental Illness or General Mental Health diagnoses under Court Ordered Treatment at our Recovery Health Center.

We offer full-time employment, Monday–Friday, 8:30am–5:30pm, with a comprehensive benefits package including 401K, PTO, and multiple medical plans. No experience required with a bachelor’s in behavioral health; new grads welcome.

Qualifications

• Duties include assisting clients with identifying treatment and recovery goals.
• Duties include completing assessments, crisis plans and individualized service plans.

Responsibilities

• Assists clients with identifying treatment and recovery goals and provides supportive services to help clients reach their goals.
• Completes assessments, crisis plans and individualized service plans.
• Works collaboratively with the clinical team to engage, educate, communicate and coordinate care with member, their family, behavioral health, medical and dental providers, community resources and others.
• Conducts outreach and follow-up of services including crisis interactions, missed appointments, and hospital discharges.
• Performs all case management functions including assessments and service planning processes.
• Pursues best practice outcomes for people with mental illness including employment, independent housing and community tenure.
