Job summary

Care-For-the-Homeless- seeks a dedicated Case Manager to provide direct case management services for homeless individuals, addressing social, mental health, substance use and medical needs to promote stability and empower clients.

The role collaborates with a multidisciplinary team to assess needs, develop treatment plans, coordinate housing applications, and support clients toward independent living with ongoing advocacy and documentation.

Qualifications

  • Bachelor's degree in Social Work, Psychology, Counseling or related field preferred
  • 3–5 years of case management experience with homeless, mentally ill, substance abuse or low-income populations preferred
  • Strong relationship-building and communication skills
  • Ability to work independently but coordinate with supervisor as needed
  • Bilingual in English and Spanish strongly preferred

Responsibilities

  • Completes intakes for new clients and orients them to the program and shelter rules
  • Reviews logbooks at shift start to ensure follow-ups are conducted
  • Collaborates with multidisciplinary staff to monitor clients' progress
  • Develops comprehensive psychosocial assessments to address client needs
  • Monitors medication compliance and addresses non-adherence
  • Establishes individualized intervention/treatment plans
  • Advocates for entitlements and services for clients
  • Provides case management including counseling, crisis intervention and referrals
  • Uses motivational interviewing and safety modalities to counsel clients
  • Facilitates groups and maintains documentation in case records and databases
  • Completes housing applications based on client needs
  • Escorts clients as needed and assists with transitions to independent living
  • Participates in weekly case conferences and staff trainings

TheCaseManager providesdirect case management services whileengaging homelessindividualswith appropriate concern for their social, mental health, substance abuse and medical problems in order to promote client stability and empower them to reach their fullest potential.

ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Completesintakes for new clients and providesanorientation to the program and shelter rules and regulations
  • Reviewslogbooks atthestart of each shift toensure that all clients follow-upsareconducted as required
  • Participatesas part of a team and regularlycollaborateswith multidisciplinary staff to monitor clients’progress
  • Developsa comprehensive psychosocial assessment to fully identify and address theclient's needs
  • Collaborateswith internal staff to monitor clients’ medication compliance and address episodes of non-adherence
  • Establishesan individualized intervention/treatment plan or strategy to promote identified goals
  • Providesadvocacy for individual clients around entitlements and services
  • Providescase management, including individual counseling, crisis intervention, conflict resolution, advocacy,psychoeducation, coordination of services and appropriate follow-up on all referrals
  • Usesmotivational interviewing and seekssafety modalities to counsel clientsto improve overalleffectiveness
  • Encouragesand providesongoing support so that each client might assume an active role in his or her owntreatment plan tothe maximum extent possible
  • Facilitatesclinical, educational, supportive and/or recreation groups to address problemareas that clientsexperience in working towards independent living
  • Maintainsrequired documentation in case records,in written and computerdatabases(SCIMS, AWARDS) andupdatesclientrecords in accordance with governmental and agency policy
  • CompletesHousing applications (both written and computer generated) for clients based on their individual needsandfunctioning
  • Escort clients as needed.
  • Facilitatesand assistswith clients' transfers into independent living
  • Participatesin weekly case conferences with both internal departments and externalproviders
  • Participatesin regularsupervision, team meetings,caseconferences and staff trainings
  • Maintainsa flexible scheduleincludingevening and weekend shifts
  • Performsother duties asit relates to case management
JOBQUALIFICATIONS
Educational Requirements
  • Bachelordegree in a Social Work, Psychology,Counselingoranotherrelated fieldpreferred
Experience
  • Three (3) to five (5) yearsofcase management experience with the homeless, mentally ill, substance abuse and/orlow-incomepopulations preferred
Skills Set
  • Strong relationship-building skills required
  • Ability to work independently but know when to raise issues tothesupervisor
  • Excellent organizational and time management skills necessary
  • Ability to effectively multi-taskandfollow upon a timely basis
  • Strong PC skills required including Word and Excel
Languages
  • Bilingual in Englishand Spanish strongly preferred
