Job summary

HOME/LIFE SERVICES INC. seeks a Case Manager to oversee client cases from intake to move-out, ensuring housing stability and wellbeing in accordance with DHS guidelines.

You will coordinate assessments, documentation, and independent living plans while collaborating with the Housing Specialist to assist clients toward exit strategies. This full-time role demands strong interpersonal, written, and organizational skills in a fast-paced environment.

Qualifications

  • Bachelor's Degree in Psychology, Social Work, Sociology, Mental Health, Nursing, Human Services, or Education Services required.
  • Minimum of 2 years’ experience working within the Social Services field.
  • Must have strong interpersonal skills and be able to communicate effectively (verbally and in writing); must be organized.

Responsibilities

  • Carries out case management activities per DHS regulations and guidelines for homeless families.
  • Completes assessments and gathers required documents (birth certificates, IDs, etc.).
  • Maintains client records by reviewing case notes and logging events.
  • Communicates clients’ progress by conducting weekly interdisciplinary meetings and evaluations.
  • Handles intakes of adults and develops an Independent Living Program (ILP).
  • Collaborates with Housing Specialist to create exit strategies and target exit dates.
