Job summary

PAM Health Specialty Hospital of Sarasota is seeking a Case Manager to coordinate care for a designated patient population within a hospital setting. You will review inpatient records to ensure appropriate resource use and determine admission levels, while coordinating discharge planning with the interdisciplinary team.

Preferred licensure in the state, BLS certification, and experience in inpatient settings (acute/IRF/LTACH) are required.

Qualifications

• Licensure in the state where the hospital resides is preferred.
• Professional disciplines may include occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or (licensed) social worker.
• Current BLS certification required.

Responsibilities

• Interdisciplinary coordination of care for a designated patient population.
• Reviews inpatient admission records to ensure proper utilization of hospital resources and admission decisions.
• Assess discharge needs and coordinate the discharge plan.
• Maintain and enhance payor relationships.
