Job summary

Qualifications

• Bachelor's degree in human services or related field with 1 year of relevant experience (or 3+ years with another degree).
• Travel up to 40% to visit members.
• Valid state driver's license and proof of personal vehicle liability insurance with 100/300/100 limits.

Responsibilities

• Assess members' health and safety needs to develop a comprehensive Member Care Plan (MCP).
• Coordinate with a Field Care Nurse to provide services in the least restrictive environment.
• Travel to members' homes for assessments and quarterly visits; maintain monthly contact by phone.
