Job summary

Telecare Corporation in Oakland, CA is seeking a Case Manager III to support individuals with serious mental illness in the community. Under direct supervision, you will coordinate services, help clients live independently, and ensure access to benefits and supports.

Shifts are Full-Time, DAYS, 8:30 AM–5:00 PM, Sunday–Thursday. The position offers a sign-on bonus of $3,000. Starting wage range is $23.00–$27.63 per hour, with geographic differentials; pay is commensurate with experience.

Qualifications

• Bachelor’s degree in Social or Human Services or equivalent experience.
• Valid and current driver’s license with personal vehicle insurance.
• Willingness to use your personal vehicle to transport clients to appointments and groups.

Responsibilities

• Serve as a primary support and case manager for individuals with mental illness and co-occurring disorders.
• Assist clients to live as independently as possible in their chosen community.
• Coordinate services and connect clients to benefits, housing, and support resources.

Education

Bachelor’s degree in Social Services or Human Services

High School Diploma or GED with 4 years direct mental health/community services
