CAMBA seeks a Case Manager II to support residents at Jamaica, NY 11432. You will maintain client confidentiality, conduct intake assessments, and collaborate with tenants to create independent living plans. This role requires strong advocacy, coordination with external services, and consistent documentation of progress toward goals.

The position offers full-time hours with comprehensive benefits and a focus on helping tenants achieve stability and self-sufficiency in the community.

Competenze

• Experience working with the Seriously and Persistently Mentally Ill (SPMI) population, and/or equivalent experience.

Mansioni

• Maintain professional relationships with clients and client confidentiality.
• Review intake and assess tenants' needs, and develop living plans.
• Create and maintain tenant files and document progress notes.
• Coordinate referrals to community resources and services.
• Escort tenants to appointments and assist with applications for benefits.
• Prepare reports for weekly/monthly/quarterly/annual reviews and participate in case conferences.
