Seven Hills Family Services, an affiliate of Seven Hills Foundation, is seeking a Case Manager II in Worcester, MA. The role oversees Shared Living/Adult Family Care programs, ensuring high-quality care and regulatory compliance.

The position features a Monday–Friday day schedule with a wage of $23.90 per hour and full-time benefits. Responsibilities include contract management, program planning, team collaboration, and case management coordination.

Qualifications

• Bachelor degree required.
• Standard driver’s license – class C or class D.
• No license required for this position.
• CPR/First Aid required.
• 1–3 years of related experience.

Responsibilities

• Manage daily operations in compliance with policies and licensing regulations.
• Oversee contract usage and funding allocations for Shared Living/AFC supports.
• Develop and implement weekly program activities and person-centered planning.
• Collaborate with team members, funding sources, families, and contractors.
• Meet regularly with the Area Director and provide monthly data updates.
• Provide case management services and coordinate support services.
• Oversee pre-placement process including recruitment, screening, and intake for providers.
• Visit each Shared Living/AFC Provider to ensure appropriate supports.
• Ensure training and compliance for providers, safeguarding rights, autonomy, safety, and health.
• Maintain accurate and up-to-date records for consumers and providers.
• Respond to correspondence within 24 hours.
• Lead and motivate staff and volunteers as a frontline supervisor.
