Responsibilities

  • Program Oversight: Manage daily operations in compliance with Seven Hills Family Services policies and licensing regulations.
  • Oversee contract usage and funding allocations for Shared Living/AFC supports.
  • Develop and implement weekly program activities and Person-Centered Planning.
  • Collaborate with team members, funding sources, families, and Shared Living Providers.
  • Facilitate meetings with funding agencies, family members, and clinical team members; provide monthly data updates.
  • Provide case management services and coordinate support services for identified individuals.
  • Visit Shared Living/AFC Providers to ensure appropriate supports are in place; coordinate onboarding.
  • Ensure training/compliance of providers; maintain required certifications and attend trainings.
  • Maintain accurate records; respond to correspondence within 24 hours; support performance objectives.
