Seven Hills Family Services, an affiliate of Seven Hills Foundation, is seeking a Case Manager II – Family Support in Athol, MA to coordinate Family Support services with individuals, families, DDS, and community partners. You will create individualized Family Support Plans to help individuals reach their personal best and support meaningful social relationships and purposeful work.

Formación

• Bachelor degree preferred.
• Standard driver’s license – class C or class D.
• No license required for this position.
• 3–5 years of related experience.

Responsabilidades

• Coordinate Family Support services in collaboration with individuals, families, DDS, and community partners.
• Create individualized Family Support Plans focused on helping individuals reach their personal best.
• Support individuals in building meaningful social relationships and engaging in purposeful work.
• Conduct home visits and maintain consistent, ongoing communication with families.
• Help families stay connected and reduce the need for out-of-home residential services.
