Seven Hills Family Services, an affiliate of Seven Hills Foundation, is seeking a Case Manager II – Family Support to coordinate services with individuals, families, DDS, and community partners. Create personalized Family Support Plans to help people reach their personal best.

The role involves home visits, helping individuals build meaningful social connections, and supporting families to stay connected and avoid out-of-home placements.

Competenze

• Bachelor degree preferred.
• 3–5 years related experience.
• Standard driver’s license required.
