Job summary

Telecare's Canyon Vista Center in Redwood City, CA seeks a part-time Behavioral Health Case Manager. Under supervision, you will support adults with serious mental illness, helping them live independently in San Mateo County. The PM shift runs Thursday to Saturday, 3:00 pm–11:30 pm, with a starting wage of $23.68/hour.

Responsibilities include creating Recovery Plans, coordinating services, and providing crisis support while ensuring HIPAA compliance and proper documentation.

Qualifications

• High School diploma or GED or equivalent with direct services experience in mental health or community services.
• CPR, CPI and First Aid certification or obtained within 60 days of employment.
• Must be 18 years or older and able to drive a personal vehicle as needed.
• Post-offer screenings including background checks and TB testing may be required.

Responsibilities

• Serve as primary support and case manager for individuals with mental illness.
• Assist tenants to live independently in the community.
• Collaborate with the Recovery Plan goals and provide required documentation.
• Transport clients to appointments and coordinate with multidisciplinary team.
• Maintain confidentiality and adhere to HIPAA policies.
