Seven Hills Foundation is seeking a Direct Support Professional II to provide one-to-one support in a private home in Massachusetts. You will help individuals with disabilities build independence, engage in meaningful activities, and maintain a safe, supportive home environment while partnering with caregivers.

The schedule includes weekday 8:45a-4:15p and weekend 8:45a-5p shifts. You will implement supports, assist with life skills, coordinate medical appointments, and document services to

Qualifications

• High school diploma or GED required.
• Experience in direct support or disability services preferred.
• Valid driver’s license class C or D1.

Responsibilities

• Provide person-centered support in a private home and the community.
• Implement Individual Support Plans, behavior supports, and therapeutic routines.
• Assist with life skills, household responsibilities, and community activities.
• Coordinate medical appointments and support overall health and wellness.
• Document services and maintain confidential records in accordance with agency standards.
