The role includes developing plans, coordinating medical and community services, advocating for rights and privacy, supervising staff, and ensuring timely documentation.

Qualifications

• CPR/First Aid certification required or ability to obtain.
• Experience coordinating clinical, community, and support services is a plus.

Responsibilities

• Coordinate, schedule, and monitor services for individuals to meet support needs.
• Provide case management and coordinate medical, clinical, and community services.
• Develop and implement person-centered plans and individual support agreements.
• Advocate for individuals’ rights, independence, privacy, and goals.
• Supervise and train staff; monitor budgets and program resources.
