Job summary

State of Arizona, Department of Economic Security (DES), Division of Developmental Disabilities (DDD) is seeking a Case Manager 2 to support Arizonans with developmental disabilities and their families. The role emphasizes individual service planning, community resources, and collaborative advocacy.

Qualifications

• Licensed RN in good standing or a BA in Social Work/ Psychology/ Counseling.
• Minimum 1 year of case management experience for elderly or disabled individuals, or 2 years in related fields.
• Must meet pre-employment and background checks and E-Verify requirements.

Responsibilities

• Drive to members' homes, schools, and program sites for needs assessment.
• Develop, write, review ISP plans within mandated timelines and coordinate referrals.
• Research community resources to inform members of options and supports.
• Facilitate meetings and collaborations with families and service providers.
